What must be included in an employer census?

Our underwriting process is most effective when we receive a full company census that is up to date to within 60 days of the requested effective date.

We require a member level census including zip codes and/or full home addresses. Zip codes are necessary; home addresses are optional but may allow for more competitive pricing.
